Patient Portal — Terms & Consent

Effective date: [EFFECTIVE DATE — set at counsel review]

What the portal is

The TeamDoctor patient portal lets you view parts of your medical record, see released test results, request appointments and refills, view bills, and exchange secure messages with your practice.

Not for emergencies

The portal and its messaging are not monitored continuously and must not be used for urgent or emergency matters. If you are experiencing a medical emergency, call 911.

Your account

Keep your password private. You are responsible for activity under your login. Tell your practice immediately if you believe your account has been accessed by someone else.

What you may see

Your practice controls what is released to the portal and when. Some results are reviewed by your care team before release. The portal is a convenience view — your complete legal medical record is maintained by your practice, and you may request a full copy from them at any time.

Consent

By activating a portal account you consent to receive communications from your practice electronically, including secure messages and account emails. You may stop using the portal at any time by asking your practice to deactivate your access. [COUNSEL: e-communications consent wording, minor/proxy-access policy.]
